What We Do
Need to fix an error on your child’s birth certificate?
Whether it’s a name, date, or missing information, we guide you step-by-step. No confusion, no wasted time.
How It Works
1. Email CHS (Center for Health Statistics): chs.amendments@oha.oregon.gov
Include: Parent name, phone, email, child’s name, date of birth, and correction needed.
2. For Parent Errors: Complete an Affidavit to Correct a Birth Certificate.
We offer FREE notarization at Umatilla County Public Health – both parents must be present if listed on the record.
3. Bring:
- Photo ID(s)
- Birth certificate(s)
$35 fee per certificate (check/money order to OHA/Vital Records)
Additional Help
We also assist with:
- Adding missing information (within 12 months of birth)
- Voluntary Acknowledgment of Paternity
- First-time name changes (second changes require court order)
First replacement certificate is free (within 6 months). Additional copies $5 each.
Act now – the sooner you start, the faster it’s fixed.
If your child is over the age of six months you can order a birth certificate from the state via phone 888-896-4988 or visit Vital Checks
